Body Contouring After Bariatric Surgery: Completing Your Transformation
For many patients at Goal BMI in Brooklyn, the journey doesn’t end when the scale stops moving. Losing 100 pounds or more through Gastric Sleeve or Gastric Bypass is a life-saving achievement. However, massive weight loss often leaves behind significant redundant, hanging skin.
Understanding the relationship between your bariatric surgery and future skin removal (like a Panniculectomy or Abdominoplasty) is essential for planning your total body transformation. Dr. Sergey Terushkin, MD, FACS, explains how these two surgical phases work together to restore your health and confidence.
Why Does Excess Skin Occur After Bariatric Surgery?
When a person carries significant excess weight for a long period, the skin’s elastic fibers (collagen and elastin) become overstretched and damaged. After a Gastric Sleeve, the fat cells shrink rapidly, but the skin often cannot “snap back” to the new, smaller body contour.
The result is a “panniculus”—an apron of skin and fat hanging from the lower abdomen. This isn’t just a cosmetic concern; it is a clinical condition that can lead to:
Intertrigo: Chronic, painful rashes and fungal infections in skin folds.
Mobility Issues: The weight of the skin can interfere with exercise and walking.
Back and Neck Pain: The “pull” of the excess weight shifts your center of gravity.
Timing Your Transformation: When Is It Time for Skin Removal?
At Goal BMI, we emphasize that patience is key to a successful result. Performing skin removal too early can lead to complications or the need for a second “touch-up” surgery if weight loss continues.
The Criteria for Candidacy:
Weight Stability: You should have a stable weight for at least 6 months.
Post-Op Duration: Typically, patients are 12 to 18 months post-bariatric surgery.
Nutritional Health: Your protein and vitamin levels must be optimal to ensure the long incisions heal properly.
Panniculectomy vs. Tummy Tuck: What’s the Difference?
One of the most frequent questions Dr. Terushkin receives is whether insurance will cover the “tuck.” It is important to understand the clinical distinction:
Dr. Terushkin specializes in documenting the medical necessity of a Panniculectomy to help patients get the functional relief they need through their insurance provider.
Real Transformations: Excess Skin Removal Results
“The journey to health doesn’t always end when the weight loss goal is achieved. For many, the final step in their transformation is addressing the functional and aesthetic challenges of redundant skin. Below, we invite you to explore our gallery of real patient outcomes following Panniculectomy and body contouring procedures performed by Dr. Sergey Terushkin. These ‘Before and After’ images provide a realistic look at how clinical expertise can restore mobility, resolve chronic skin issues, and finalize a patient’s new silhouette. Each case is unique, and while these photos demonstrate the high standard of care at Goal BMI, individual results may vary based on a patient’s unique anatomy and healing process.”
Frequently Asked Questions
Q: Does insurance cover skin removal after a Gastric Sleeve? A: Many insurance plans cover a Panniculectomy if we can document “functional impairment,” such as chronic rashes that don’t respond to creams or significant interference with walking.
Q: Is it safe to have skin removal after bariatric surgery? A: Yes, provided your weight is stable and your nutrition is excellent. Dr. Terushkin’s 15+ years of experience in both fields ensures a safe surgical transition.
Q:Can I have skin removal and hernia repair at the same time? A: Yes! This is very common. Dr. Terushkin often repairs umbilical or ventral hernias during the same session as skin removal, reducing your total time in the operating room.
Q: Will I have a scar after skin removal? A: Yes. Skin removal requires a long horizontal incision. However, Dr. Terushkin places it very low on the abdomen so it can be hidden by most undergarments or swimwear.
Q: How much weight is removed during a Panniculectomy? A: Depending on the patient, the removed skin and fat can weigh anywhere from 5 to 15 pounds, providing immediate relief from back strain.
Q: What is the recovery time for skin removal? A: Most patients require 2 to 4 weeks before returning to light work, and 6 to 8 weeks before resuming heavy exercise.
Q: Why choose Dr. Terushkin for both surgeries? A: By seeing a surgeon who understands both the bariatric “input” and the reconstructive “output,” you get a seamless, coordinated care plan from a specialist who knows your entire medical history.

I scheduled a consultation with Dr. Terushkin after struggling with a 10-month weight loss plateau, 50 lbs away from my goal following bariatric surgery. I was desperate for other options. I appreciated how direct he was, got straight to the point and laid out a clear plan.
Seven months later, I’ve reached my goal weight flawlessly. The clinic is beautiful, spotless, and equipped with very friendly, supportive staff. I’m so glad I visited last autumn. It was truly life-changing. Highly recommend.

Almost 3-year update: today, I have passed my goal Weight of 200 LBS and am at 175 LBS. I lost a total of 145 LBS—still no issues or Complications. One of the best Decisions I made was picking Goal BMI.

Amazing Experience!
I started my process back in March and went in a little skeptical, hoping this would be the right place for me. I met with Dr. Alex and from the first appointment I knew I was in good hands. He is a great doctor, very friendly, real, and honest. He explained what would work best for me and guided me every step of the way.
The front desk staff is always nice and welcoming and the office is clean and organized. I also had some online appointments since I couldn’t always go in person and the process was super smooth. They work closely with other offices and specialists who already know what you’re there for which made everything easy and efficient when you go for your pre op requirements or clearances.
I had my gastric sleeve surgery on Monday 10/20 and Dr. Alex was amazing. He met with me before surgery, after surgery, and again on the day of discharge. That level of care meant a lot.
Today is day six post-op and I’m feeling really good. The first few days were a little painful which is totally normal after surgery, but I’m healing very nicely. I’m already recommending this office to my friends who are interested in doing the same.
100% recommend Dr. Alex and his team!
